At Mark Jaffee DDS, it is our mission to share with you how important dental treatment is. Financial considerations should not be an obstacle to obtaining this important part of your healthcare.

 
 
  • We accept Visa, Mastercard, Discover, American Express, cash or check for
    payment.

  • If you have dental insurance, we will assist you in completing and submitting claims to your carrier. You are ultimately responsible for contacting your dental insurance company directly and verifying that we are in your network plan that your employer has chosen for you. Insurance companies have many levels of plans that are offered to employers and we ask that you contact them directly to verify that we are in your network. This also brings up another question – some employers provide plans that have in/out of network coverage. Here are a few questions to ask your carrier when calling:

    1. Does my plan cover in/out of network providers?

    2. If so, is the coverage the same across the board for an in or out of network provider for preventative, basic & major services?

    We are currently contracted with Delta Dental Premier, Cigna, Guardian and Aetna. Again, since they have different levels of plans, we do encourage you to contact them directly to verify your benefits.

    We are happy to assist you in providing a predetermination to your carrier which will provide you with an estimate for treatment. Estimates are not a guarantee of payment. We will do everything that we can to maximize your insurance benefits for you. You are responsible for payment for services after insurance has paid.

  • Don’t have insurance? No problem! We accept Care Credit. Care Credit is like any other credit card, you must go through an online application and we can process an application with you at the office or over the phone. Care Credit offers 12 months plans at 0% interest **as long as make monthly payments on time and before the promotional 12 months expires**. If you have a larger treatment plan, they offer extended payment plans at fixed a APR. Visit their website for any other questions.
